3C589B - FT4000 (Canon OEM)

Problem: NDIS driver won't initialize with Card Services present.

Solution: Remove the definition of the adapter from the PCM.INI file. Exclude IRQ 5 in the PCM.INI to avoid a conflict with the on-board sound card:
[DEFAULT.RESOURCES]
Exclude_irq=0x05

The NDIS driver now loads at Phoenix Card Service values of IRQ 11, I/O 100.
